Geriatrics clinicians involved in the consultation service are able to address issues common in the elderly, such as incontinence, Hospital-Induced Delirium, Dizziness, Dementia, Polypharmacy (use of multiple medications by a patient), Falls, Sleep Disorders, Elder Abuse, Palliative Care, and Discharge Planning

Geriatric Care Management is the process of planning and coordinating care of the elderly and others with physical and/or mental impairments to meet their long term care needs, improve their quality of life, and maintain their independence for as long as possible.

At Dr. Chaudhry office, we are changing the way we look at, think about, and experience aging. Our expert Geriatricians provide clinical services and programs that encourage healthy aging, treat patients with chronic illnesses, and provide support and education for family members and caregivers.
